You have a choice between two medical plan options: the PPO Plan and Consumer Driven Health Plan (CDHP). Both are available under the Aetna Open Choice POS II network and offer the same services and quality of care. Preventive care is covered at 100% with in-network providers in both plans. The difference is in when and how you pay.
No matter which plan you choose, your in-network medical costs are capped at $3,500 or $7,000 depending on how many people you cover. A Health Savings Account or Flexible Spending Account through HSA Bank can help cover these expenses depending on your plan guidelines.

Accident insurance
Payments are made directly to you for more than 150 covered events such as fractures, dislocations or burns, regardless of any benefits that your medical plan may pay.
Hospital indemnity insurance
Provides a direct lump-sum payment if you or a family member become hospitalized.
A flat amount is paid for the day that you are admitted to a hospital and a per-day amount is paid for each additional day of a covered stay.
Critical illness insurance
Provides a lump-sum payment, made directly to you, upon diagnosis of serious conditions like heart attack, cancer and stroke.
MetLife legal
Gives you access to a network of attorneys to help when you’re buying a home, planning for your family’s future, caring for aging parents and other key times in your life.
Digital estate planning tools allow for quick creation of wills and powers of attorney with no copays or deductibles when using network attorneys.
Aetna dental
Our dental plan, administered by Aetna, offers you one of the largest provider networks in the country. You can visit any provider, but you’ll pay less when you stay in-network.
VSP vision
The vision plan is offered through Vision Service Plan (VSP), which provides a network of providers who offer discounted fees and wholesale prices for routine eye exams, lenses and frames. Should you use a non-network provider, the plan partially reimburses your costs.
Teladoc
Save time and money by addressing medical conditions such as flu, allergies or skin issues via phone or video consultations any time of day. Medical experts can help you find to specialists and provide a quick, as well as an informed, second opinion about a recommended surgery, new treatment or serious illness. Available for employees and their dependents covered under the LYB medical plan.
Wondr Health
Helps you to identify behaviors and suggests modifications to help you manage your weight. Available for employees and their dependents covered under the LYB medical plan.
Hinge Health
Provides virtual at-home exercise therapy, one-on-one coaching, and a personalized curriculum to address issues with your neck, back, shoulders or other joints. Participants report significant reductions in pain, depression and surgeries. Available for employees and their dependents covered under the LYB medical plan.
Progyny
Provides an inclusive family-building benefit with coverage for both medical care and prescriptions. Employees and their spouses who are covered under the LYB medical plan can access a high-quality network of fertility specialists and a dedicated Patient Care Advocate for support. Coverage also includes doula and menopause support.
401(k) plan
LYB partners with Fidelity to offer a 401(k) plan to help you save for your retirement. You are eligible from day 1 and the company matches the first 6% of your contribution dollar for dollar. Choose pre-tax, Roth or after-tax contributions. You are immediately vested in both your contributions and the company match.
Student debt 401(k) match
With LYB Student Debt 401(k) Match, your student debt payments will be used as if they’re 401(k) contributions, and count towards your yearly company match. If you contribute to your LYB 401(k) plan, you’ll receive your regular match payments according to your plan’s rules. Any additional match accrued from your student debt payments will be contributed to your retirement plan after the plan year ends.
Pension plan
You are automatically enrolled in the LYB Pension Plan upon your date of hire. The company funds the pension plan and shoulders all investment risk. Pay credits are calculated as 5% of your base pay and applied quarterly to your balance along with interest credits. You are vested in the Pension Plan after three years of service.

The LYB Employee Stock Purchase Program (ESPP) with Morgan Stanley is an easy way to purchase LYB shares at a 10% discount off the fair market value of shares on either the first or last trading day of each calendar quarter, whichever is less. Simply elect to contribute between 1 to 10% of your salary and your contributions will be taken through payroll deductions throughout the calendar quarter.
